Campaign Achievement bugs

I recently finished "Through Mud and Blood" and got all the codexes, challenges there by spending so much time and it did not give me achievement. And I recently completed "Friends in High Places" just to get no achievement. How can I fix this glitch or issue

go into war stories and click on the mission remeber us it fixed my solution same thing happended to me when i completed the game on hard mode

and watch the whole cutscene no skip
