Spear of Destiny is a prequel, as are "Nocturnal Missions" (episodes 4-6). Chronologically, events of episode 3 are the latest as far as Wolfenstein 3D is concerned.

Simply speaking, because Doom and Quake were so successful that id Software never returned to the topic of Wolfenstein. As for other developers, I think that the idea of killing Adolf was so impactful in terms of plot that they preferred to reset it.

There was, at some point.
Early in development, Rise of the Triad was intended to be Wolfenstein 2. There's a few things leftover like Nazi enemies/aesthetics, and German weapons like an MP40. However, id Software wanted to cancel the project so Apogee turned it into a standalone game instead.
